A Custody and Control Form (CCF) or chain of custody form is a document which is used to track a specimen in the drug testing process and to ensure that the specimen is handled in accordance with testing guidelines. It is an important part of the drug testing process and enables tracking of the specimen from the moment it is collected from the donor until the results are reported and interpreted.
A federal chain of custody form is used for drug testing for Department of Transportation (DOT) Exams. It consists of five copies which are used by different individuals involved in the testing process. Throughout the process of testing the specimen, each person involved records their own actions regarding handling the specimen.
What changes have been made?
The revised CCF was approved for use beginning on September 1, 2020 and can be used currently. Starting on August 30, 2021, the revised Custody and Control Form must be used. If an older version of the CCF is used after this date, it will be required to include a Memorandum for the Record from the collector.
Most of the changes adopted for the revised CCF are to accommodate the use of oral fluid specimens in addition to urine specimens for the federal drug testing program. While oral fluid drug testing has been approved for use by some agencies, it has not yet been authorized for the DOT’s drug testing program. In addition, there have also been updates to some steps of the form to include additional clerical information such as the donor’s CDL number, and email address.
What do I have to do for my employees?
If you are sending your DOT employees for a drug screen with a paper filed CCF after August 30, 2021 you have to ensure that you have the appropriate revised paperwork on hand.
