OSHA Questionnaire

The OSHA Questionnaire is required in workplaces in which employees may be exposed to hazardous airborne contaminants. A Respirator Evaluation Questionnaire is conducted prior to a Respirator Fit Test and requires the employee to answer questions related to their health history and work conditions. The questionnaire is reviewed with a health professional who can then recommend if a physical exam should be performed in conjunction with the fit test.
